STLP Shipping Request
This form is valid for Spring 2024 equipment loans from the STLP office location in Kane Hall (KNE). Equipment items eligible for shipping are denoted with "(UPS)" on the equipment reservations page. If your equipment reservation is for a different STLP office, please cancel and reserve an eligible item from KNE. This form will close to new submissions at 5:00pm on Wednesday, June 5th, 2024.

US domestic shipping is provided at no cost to students (no international shipping available). Equipment items will be shipped UPS Ground for in-state (Washington) addresses and UPS 2nd Day Air for out-of-state addresses. Please complete the form below as follows:
  • Enrolled Students: If you're located outside the City of Seattle and are unable to physically checkout equipment from STLP, you may request shipping of your equipment to a US residential address (City of Seattle, PO boxes, and international addresses will not be accepted). You have the option of returning your equipment in person or using this form to request a prepaid return shipping label.
  • Admitted Students: If you're participating in First Year Programs (FYP) Advising & Orientation (A&O), shipping is required (no in person checkout available). Your equipment will be shipped a few days prior to your scheduled A&O date and will automatically include a prepaid return shipping label. Equipment reservations associated with FYP A&O are provided for a period of seven (7) business days only.
STLP staff will follow up via email and provide a tracking number once your item has shipped. Not all equipment items are eligible for shipping. Ineligible items must be picked up from/returned to STLP in person. For specific questions regarding shipping, please email stlp@uw.edu or call/text 206-685-6090.
